Forms Completion via Patient Portal
We are delighted to announce that we now have the facility for our patients to complete many of the necessary pre-appointment forms online at your own convenience.
By using the Patient Portal facility provided by our Software of Excellence medical records system, we can now offer the option for several forms including medical history and COVID screening questions to be completed prior to your visit. This will mean less time needing to be spent at the practice, and a further step towards reducing unnecessary contacts as we look to make your journey whilst with us as “Covid secure’ as possible.
When you have an appointment booked, you will be sent a link via your chosen communication method. This link is secure and individual to you. On pressing the link you will be taken to the SOE Patient Portal where you will be prompted to log in with your details.
You will then be guided through the completion of any requested forms and asked to submit these responses.
The completed forms will then automatically came back to us via the portal and will be linked directly with your clinical record, ready for review and discussion at your following visit.
